Forms & Policies
The following forms are designed to help us work together more easily and provides your therapist with important information for your massage.
All new clients must complete a Health History Form, New Client Intake and Informed Consent prior to their first appointment. Please print, complete and bring these with you to your first session. Any time you have a medical change please complete and bring in an updated Health History Form.
Payment: Payment is due at time of service and can be made by cash or personal check.
Return Check Policy: A $39 fee will be assessed to the Client if a check is returned for non-sufficient funds. All future appointments will need to be be paid for in cash.
Late Policy: Clients arriving within 15 minutes of their start time will receive the remaining time of their session and pay full price. Clients arriving after 15 minutes or no-shows will forgo services and be responsible for the full fee; future appointments may require a deposit.
Cancellation Policy: 24 hour notice of cancellation must be provided. Cancellations within 24hrs will be assessed 25% of the service fee & shall be charged to the Client for failure to provide 24 hours notice. No-shows will forgo services and will be responsible for the full fee.
Cancellation/Appointment Change Policy for 3rd Party Vendor Programs: Discount Programs include, but are not limited to, purchases made via 3rd party vendors such as LivingSocial, Groupon and similar discount programs. If the service was purchased via a Discount Vendor 48 hour notice must be given for cancellations or appointment changes. Failure to provide 48 hours notice shall result in the forfeiture of the purchased voucher.
This form gives an overview of your health history and medical conditions that may affect the massage session.
This form provides general contact information as well as informing the client of policies and procedures.
This form allows a parent or legal guardian to give permission to perform massage on a minor client.
If you'd like to set up massage for your company as a one-day treat or as a recurring benefit, this contract lays out the details of our agreement.
This is a standard release of liability for our massage therapy work together.
